Wat betreft economische groei en ontwikkeling van de werkloosheid heeft de Nederlandse economie het sinds 1973 slechter gedaan dan andere OECD-landen. Op de vraag naar de oorzaken van die slechte prestatie zijn in het verleden uiteenlopende antwoorden gegeven door o.m. Bomhoff en Clavaux. Ook zijn er diverse wegen aangegeven om op te rukken naar een betere positie. In dit artikel presenteren de auteurs de resultaten van een internationale doorsnee-analyse om de verschillen in economisch succes tussen landen met behulp van een (...) eenvoudig neo-klassiek macro-economisch model te verklaren. Dit biedt de mogelijkheid om te traceren wat de rol van de exportgroei, de groei van de bevolking, de wisselkoers, de verhouding tussen de binnenlandse en de buitenlandse prijsontwikkeling en de investeringsquote voor verschillen in economische ontwikkeling is geweest. Vooral de uitvoer gecorrigeerd voor de omvang van de bevolking, de investeringsquote en de nominale wisselkoers komen als belangrijke verklarende variabelen naar voren. Op grond van hun analyse komen de auteurs tot de conclusie dat een beleid gericht op het verbeteren van de Nederlandse economische prestatie op lange termijn zich vooral zou moeten concentreren op het benutten van exportkansen en op een hoog investeringsniveau ter bevordering van technologische vernieuwing en daarmee verbetering van de concurrentiepositie. Daarbij moet worden voorkomen dat revaluatie van de gulden het exporteffect teniet doet. (shrink)

Een vrije en veerkrachtige samenleving ontstaat niet vanzelf en blijft niet vanzelf bestaan. In dit themanummer verkennen we de publieke onderwijstaak van het hoger beroepsonderwijs om studenten te onderwijzen de gemeenschappelijke wereld én het publieke gesprek over die wereld centraal te stellen. Met dit themanummer willen wij iets tegenover het vigerende marktdenken zetten. We laten ons hierbij inspireren door het gedachtegoed van Hannah Arendt (1906–1975). Ze heeft uitgebreid geschreven over ‘het publieke’. Ze verbindt de inzet van wetenschappelijke kennis en technologie (...) met het politieke vraagstuk: willen we daadwerkelijk leven in de wereld die we aan het scheppen zijn? Arendts gedachtegoed biedt volop handvatten om op verschillende niveaus van het hoger beroepsonderwijs deze taak te doordenken en invulling te geven. (shrink)

Tussen zijn veertigste en zijn zestigste levensjaar was Jan Hendrik van den Berg (1914) een uitermate succesvol en populair auteur. Boeken van zijn hand, zoals Metabletica (1956) en Medische macht en medische ethiek (1969), waren ongekende bestsellers. Hij was de Nederlandse vertegenwoordiger van een belangrijke Europese stroming in de filosofie: de historische fenomenologie. In de jaren zeventig raakte hij echter in conflict met zijn tijd. Terwijl de Nederlandse publieke opinie een wending naar links doormaakte, bond Van den Berg de strijd (...) aan tegen verloedering, hooliganisme en anarchisme. Dit ging ten koste van zijn populariteit, maar niet van zijn productiviteit, noch van zijn innovativiteit. Tot op de dag van vandaag werkt hij aan een indrukwekkend , panoramisch oeuvre. Hij is de belangrijkste Nederlandse filosoof sinds Bolland. -/- Deze studie wil het werk van Van den Berg reconstrueren, situeren en beoordelen. Reconstrueren door de methodische en thematische coherentie ervan zichtbaar te maken. Situeren door het te confronteren met dat van andere, verwante auteurs zoals Husserl, Freud, Bachelard en Foucault. Beoordelen door na te gaan in hoeverre Van den Berg de inzet van zijn project, zowel inhoudelijk als methodologisch, waarmaakte. -/- Het werk van Van den Berg is geen curiosum, maar opvallend actueel. Hij onderzoekt het begin, de aanvang van wetenschappelijke en maatschappelijke ontwikkelingen waarvan het belang in de huidige situatie met nadruk zichtbaar wordt. Zijn beeldende stijl wekt de geschiedenis van de wetenschap tot leven, maar werd vaak als ‘anekdotisch’ afgedaan; zijn talent voor polemiek lokte soms felle reacties uit en zijn methodologische flexibiliteit maakte hem een gemakkelijk doelwit van academische kritiek. Aan de positieve kant van de balans staan echter zijn intellectuele enthousiasme en zijn internationale faam. Zijn werk verdient een rehabilitatie. (shrink)

This chapter discusses the comic potential that originates in the way players of digital games take on the dual position of being at once a played self that is internal to the gameworld and a playing self that perceives this world from the outside. I first describe the comic attitude as it is defined within philosophy: as an attitude of distanced and dispassionate reflection towards an incongruity. I then show how the dual position of players during gameplay not only is (...) characterized by incongruities, such as contradictions between the apparent seriousness and the ultimate triviality of in-game actions, but also allows players to dispassionately reflect on these incongruities. I thus argue that digital gameplay entails the inherent possibility of turning players into both a comic object and a laughing subject. (shrink)

In this chapter, I investigate the imaginary boundary between the actual world and fictional gameworlds by focusing on videogame situations in which this fourth wall is foregrounded or broken. For this purpose, I first define the videogame experience as a self-involving, interactive fiction experience, based on Kendall Walton’s account of fiction (1990). I then describe how, in the current academic discourse on games, it is often claimed that the concept of fourth wall breaks cannot be applied to videogames due to (...) their inherent interactivity. Within game studies, the consensus seems to be that the boundaries between the real and the fictional world are always already blurred in videogame experiences. This chapter instead shows how using interactive, digital technologies to represent fictional worlds does not necessarily complicate the conceptualization of the fourth wall, but rather reveals new ways in which it can be broken. More precisely, this chapter discusses how appreciators of videogames can not only actively participate in fourth wall breaks, but are also uniquely able to initiate these breaks themselves. -/- . How can we be moved by the fate of Anna Karenina? By asking this question, Colin Radford introduced the paradox of fiction, or the problem that we are often emotionally moved by characters and events which we know don’t really exist (1975). A puzzling element of these emotions that always resurfaced within discussions on the paradox is the fact that, although these emotions feel real to the people who have them, their difference from ›real‹ emotions is that they cannot motivate (...) us to perform any actions. The idea that actions towards fictional particulars are impossible still underlies recent work within the philosophy of fiction (cf. Matravers 2014, 26 sqq.; Friend 2017, 220; Stock 2017, 168). In the past decennia, however, the medium of interactive fiction has challenged this idea. Videogames, especially augmented and virtual reality games, offer us agency in their fictional worlds: players of computer games can interact with fictional objects, save characters that are invented, and kill monsters that are clearly non-existent within worlds that are mere representations on a screen. In a parallel to Radford’s original question, we might ask: how can we be moved to shoot zombies, when we know they aren’t real? The purpose of this article is to examine the new paradox of interactive fiction, which questions how we can be moved to act on objects we know to be fictional, its possible solutions, and its connection to the traditional paradox of fictional emotions. This essay seeks to see if one side or the other in the hunting debate gets more purchase if we first ask what gives the state the moral right to promote sport hunting when the practice is in deep decline. We look at the dominant economic and political reasons for state support, none of which settle the moral matter. We then look at various state appeals to moral justification (ethical hunting, the right to hunt, the value of heritage, etc.) and (...) determine that they beg the same prior questions that hunters beg--questions about the new scientific understanding of animal cognition and about the ideal individual and society in the 21st C. Neither side in the philosophical debate is helped by state promotion of hunting, given that anti-hunters also often dispense with the deeper questions and the relevant science. Conclusion: the debate should be based on the new scientific understanding of animals' capacities and on current ideals of the individual and society. State policy should reflect that. -/- thehe state should see that it is on slippery ground when it devises programs to gratify hunters and to encourage the innocent, the reluctant, and the lapsed to take up the practice. How to account for the power that art holds over us? Why do artworks touch us deeply, consoling, transforming or invigorating us in the process? In this paper, we argue that an answer to this question might emerge from a fecund framework in cognitive science known as predictive processing (a.k.a. active inference). We unpack how this approach connects sense-making and aesthetic experiences through the idea of an ‘epistemic arc’, consisting of three parts (curiosity, epistemic action and aha experiences), which we (...) cast as aspects of active inference. We then show how epistemic arcs are built and sustained by artworks to provide us with those satisfying experiences that we tend to call ‘aesthetic’. Next, we defuse two key objections to this approach; namely, that it places undue emphasis on the cognitive component of our aesthetic encounters—at the expense of affective aspects—and on closure and uncertainty minimization (order)—at the expense of openness and lingering uncertainty (change). We show that the approach offers crucial resources to account for the open-ended, free and playful behaviour inherent in aesthetic experiences. As artefacts, the worlds of digital games are designed and developed to fulfil certain expressive, functional, and experiential objectives. During play, players infer these purposes and aspirations from various aspects of their engagement with the gameworld. Influenced by their sociocultural backgrounds, sensitivities, gameplay preferences, and familiarity with game conventions, players construct a subjective interpretation of the intentions with which they believe the digital game in question was created. By analogy with the narratological notion of the implied author, we call the (...) figure to which players ascribe these intentions ‘the implied designer’. In this article, we introduce the notion of the implied designer and present an initial account of how appreciators ascribe meaning to interactive, fictional gameworlds and act within them based on what they perceive to be the designer’s intentions. Value Sensitive Design (VSD) is an established method for integrating values into technical design. It has been applied to different technologies and, more recently, to artificial intelligence (AI). We argue that AI poses a number of challenges specific to VSD that require a somewhat modified VSD approach. Machine learning (ML), in particular, poses two challenges. First, humans may not understand how an AI system learns certain things. This requires paying attention to values such as transparency, explicability, and accountability. Second, ML (...) may lead to AI systems adapting in ways that ‘disembody’ the values embedded in them. To address this, we propose a threefold modified VSD approach: 1) integrating a known set of VSD principles (AI4SG) as design norms from which more specific design requirements can be derived; 2) distinguishing between values that are promoted and respected by the design to ensure outcomes that not only do no harm but also contribute to good; and 3) extending the VSD process to encompass the whole life cycle of an AI technology in order to monitor unintended value consequences and redesign as needed. We illustrate our VSD for AI approach with an example use case of a SARS-CoV-2 contact tracing app. (shrink)

Frans H. van Eemeren y Peter Houtlosser reaccionaron contra una separación estricta entre dialéctica y retórica en varios trabajos, pero uno de los más inspiradores es sin duda su artículo “William the Silent’s argumentative discourse” (1998) presentado en la ciudad de Ámsterdam con ocasión de la cuarta conferencia organizada (...) por la International Society for the Study of Argumentation (ISSA). Sobre este tema siguieron investigando para presentar un análisis de la perspectiva dialéctica inherente al caso de Guillermo I de Orange en un congreso de la Japan Debate Association en Tokio (van Eemeren y Houtlosser, 2000). El estudio y análisis de las argumentaciones cotidianas entendidas como interacciones discursivas e intencionales encaminadas a dar cuenta de algo con el fin de lograr que aquello que se sostiene sea aceptado, sería inconcebible sin la aparición de la teoría de los actos de habla de Austin (1962), la propuesta de Searle (1969), el trabajo de Grice sobre la teoría de la conversación (1975) y el importante estudio sistemático de Hamblin sobre el argumento falaz (1970). Como una reelaboración de dichas (...) obras cabe entender la teoría pragmadialéctica de la argumentación propuesta inicialmente por Frans H. van Eemeren y Rob Grootendorst (1984). Algunos autores (Blair, 2006: 11; Woods, 1992) creen ver en ella, más que una teoría, una amalgama de varias teorías y una particular versión de la teoría pragmadialéctica de la argumentación, entendiendo ésta bajo una acepción más general. Emotions Caused by Untrue Propositions: A Broader View of the Paradox of Fiction Ever since Colin Radford wrote his article ‘How Can We Be Moved by the Fate of Anna Karenina?’ in 1975, philosophers have tried to solve the so-called paradox of fiction, or the question how we can be moved by objects of which we know they don’t really exist. What is striking about discussions on the paradox of fiction is that they often present fictional works as collections of (...) untrue statements and focus on the content of these works, without regard for their form or mode of presentation. In this paper, I argue for a re-examination of the paradox of fiction, in which the work of fiction is treated as a complex of form and content. In this regard, I propose to expand Peter Lamarque’s thought theory, which he explicitly formulated to solve the paradox of fiction, with his later developed concept of opacity, which states that the content of the imaginings we form when reading fiction are fundamentally connected to the specific descriptions within the fictional work. Variants of this property have been noticed for some time in logic programming and in linear programming. We show that a first-order theory has the Independence Property for the class of basic formulas provided it (...) can be axiomatized with Horn sentences. The existence of free models is a useful intermediate result. The independence Property is also a tool to decide that a sentence cannot be deduced. This Insight examines the stance the EU should adopt towards the Russian invasion of Ukraine on the basis of the political thought of Immanuel Kant, Thomas Hobbes, and Niccolò Machiavelli. Taking as its starting point Josep Borrell’s comment that “we are too much Kantians and not enough Hobbesians” at the 2022 EU Ambassadors’ Conference, this Insight offers a revisionist interpretation of both Kant and Hobbes while suggesting Machiavelli as a third possible inspiration for EU external action. Although he is often (...) portrayed as a proto-“realist” intent on increasing state power, Hobbes in reality favours stability above all else and would therefore presumably not support a more “aggressive” foreign policy. Kant, on the other hand, has traditionally been seen as more of a philosophical idealist, but his political philosophy in fact supports a more assertive conduct by states in their external relations. Both of these philosophers are thus quite different from how Borrell portrays them. The Insight also introduces the perspective of a third thinker, Machiavelli, whose philosophy suggests that the EU should adopt a pragmatic diplomatic strategy, forging alliances, supplying Ukraine with weapons, and maintaining strong ties with allies. In Logique du Fantasme, Lacan argues that the compulsion to repeat does not obey the same discharge logic as homeostatic processes. Repetition installs a realm that is categorically different from the one related to homeostatic pleasure seeking, a properly subjective one, one in which the mark “stands for,” “takes the place of,” what we have ventured to call “an event,” and what only in the movement of return, in what Lacan calls a “thinking of repetition,” confirms and ever reconfirms this (...) point of no return, which is also a qualitative cut and a structural loss. The kind of “standing for” Lacan intends here with the concept of repetition is certainly not something like an image or a faithful description. No, what Lacan wishes to stress is that this mark is situated at another level, at another place, it is “entstellt,” and as such, it is punctually impinging upon the bodily dynamics without rendering the event, without having an external meta-point of view, but cutting across registers according to a logics that is not the homeostatic memory logics. This paper elaborates on this distinction on the basis of a confrontation with what Freud says about the pleasure principle and its beyond in Beyond the Pleasure Principle, and also takes inspiration from Freud’s Project for a Scientific Psychology. We argue that Lacan’s theory of enjoyment takes up and generalizes what Freud was after in Beyond the Pleasure Principle with the Wiederholungszwang, and pushes Freud’s thoughts to a more articulated point: to the point where a subject is considered to speak only when it has allowed the other, through discourse, to have impacted and cut into his bodily pleasure dynamics. A recently proposed model of sensory processing suggests that perceptual experience is updated in discrete steps. We show that the data advanced to support discrete perception are in fact compatible with a continuous account of perception. Physiological and psychophysical constraints, moreover, as well as our awake-primate imaging data, imply that human neuronal networks cannot support discrete updates of perceptual content at the maximal update rates consistent with phenomenology. A more comprehensive approach to understanding the physiology of perception (and experience at (...) large) is therefore called for, and we briefly outline our take on the problem. (shrink)

The editors cull the works of 11 noted French and German philosophers for their contributions to the debate about what animals are like and how we should relate to them. Each selection gives the gist of the philosopher's view followed by a noted scholar's comments. The result, as Peter Singer notes in his merciless Foreward, is that most of the Continentals have had almost nothing of interest to say on the topic.

Drawing from narratology and design studies, this article makes use of the notions of the ‘implied designer’ and ‘ludic unreliability’ to understand deceptive game design as a specific sub-set of transgressive game design. More specifically, in this text we present deceptive game design as the deliberate attempt to misguide players’ inferences about the designers’ intentions. Furthermore, we argue that deceptive design should not merely be taken as a set of design choices aimed at misleading players in their efforts to understand (...) the game, but also as decisions devised to give rise to experiential and emotional effects that are in the interest of players. Finally, we propose to introduce a distinction between two varieties of deceptive design approaches based on whether they operate in an overt or a covert fashion in relation to player experience. Our analysis casts light on expressive possibilities that are not customarily part of the dominant paradigm of user-centered design, and can inform game designers in their pursuit of wider and more nuanced creative aspirations. This article discusses the moment in which normative ideas about aging and reproductive embodiment became conceptually linked in the mid-nineteenthcentury medicalization of menopause. The reading centers on the first English book-length publication on menopause, written by E. J. Tilt in 1857, and Foucault’s concept of the medical gaze. I analyze mechanisms of observing, conceptualizing, and treating the body in relation to time and discuss their function in affirming and reworking social norms of age and gender. In doing so, I highlight (...) the political work implicit in contesting conceptualizations of female reproductive bodies, their age-specific pathologies, and directives of surveillance employed in discourses surrounding women’s reproductive health. Philip's interest is specifically aimed toward the ethics of Transhumanism and depictions of Transhumanism in works of fiction. On the other hand, Andrew finds himself more focused on religious behavior in the technological world. While the two perspectives might not seem that close, there is certain to be an overlap in Andrew and Philip's shared understanding of how technological phenomena play (...) a crucial role concerning religion and society. In the last few years, a remarkable convergence of interests and results has emerged between scholars interested in the arts and aesthetics from a variety of perspectives and cognitive scientists studying the mind and brain within the predictive processing (PP) framework. This convergence has so far proven fruitful for both sides: while PP is increasingly adopted as a framework for understanding aesthetic phenomena, the arts and aesthetics, examined under the lens of PP, are starting to be seen as important windows (...) into our mental functioning. The result is a vast and fast-growing research programme that promises to deliver important insights into our aesthetic encounters as well as a wide range of psychological phenomena of general interest. Here, we present this developing research programme, describing its grounds and highlighting its prospects. We start by clarifying how the study of the arts and aesthetics encounters the PP picture of mental functioning (§1). We then go on to outline the prospects of this encounter for the fields involved: philosophy and history of art (§2), psychology of aesthetics and neuroaesthetics (§3) and psychology and neuroscience more generally (§4). This is review of the book "Rightful Relations with Distant Strangers: Kant, the EU, and the Wider World" by Aravind Ganesh, which discusses the relevance of Immanuel Kant's legal philosophy for the European Union's exercise of extraterritorial jurisdiction. The book explores this issue from the perspectives of public international law and private law theory as well.

Autism Spectrum Condition presents a challenge to social and relational accounts of the self, precisely because it is broadly seen as a disorder impacting social relationships. Many influential theories argue that social deficits and impairments of the self are the core problems in ASC. Predictive processing approaches address these based on general purpose neurocognitive mechanisms that are expressed atypically. Here we use the High, Inflexible Precision of Prediction Errors in Autism approach in the context of cultural niche construction to explain (...) atypicalities of the relational self, specifically its minimal, extended, and intersubjective aspects. We contend that the social self in ASC should not be seen as impaired, but rather as an outcome of atypical niche construction. We unpack the scientific, ethical, and practical consequences of this view, and discuss implications for how the challenges that autistic persons face should be approached. (shrink)

Background: Several countries have recently changed their model of consent for organ donation from opt-in to opt-out. We undertook a systematic review to determine public knowledge and attitudes towards these models in Europe. Methods: Six databases were explored between 1 January 2008 and 15 December 2017. We selected empirical studies addressing either knowledge or attitudes towards the systems of consent for deceased organ donation by lay people in Europe, including students. Study selection, data extraction, and quality assessment were conducted by (...) two or more reviewers independently. Findings: Awareness of the consent model was lower in opt-out countries than in opt-in countries. A majority of the public agrees with opt-in, regardless of the law in their own country. There are ambivalent attitudes regarding the opt-out system. The public tend to prefer opt-in and mandatory choice over opt-out when several options are offered. Interpretation: The assumption that people in opt-out countries are aware of the legal requirements to be excluded from the pool of potential donors is not supported by the results of this review. This is a concern, since ignorance hinders people's autonomy regarding organ donation decision-making. Higher awareness of consent model in opt-in countries may reflect greater efforts to inform the public through campaigns to motivate donation. Legal moves towards opt-out are at odds with people's expressed preferences. Main limitations of this review are the lack of data from some countries, study population heterogeneity, and methodological shortcomings. (shrink)

In this paper we examine to what extent the lengths of the links in an animated articulated figure can be changed without the viewer being aware of the change. This is investigated in terms of a framework that emphasizes the role of attention in visual perception. We conducted a set of five experiments to establish bounds for the sen-sitivity to changes in length as a function of several parameters and the amount of attention available. We found that while length changes (...) of 3% can be perceived when the relevant links are given full attention, changes of over 20% can go unnoticed when attention is not focused in this way. These results provide general guidelines for algorithms that produce or process character motion data and also bring to light some of the potential gains that stand to be achieved with attention-based algorithms. Goal: To assess public knowledge and attitudes towards the family’s role in deceased organ donation in Europe. -/- Methods: A systematic search was conducted in CINHAL, MEDLINE, PAIS Index, Scopus, PsycINFO, and Web of Science on December 15th, 2017. Eligibility criteria were socio-empirical studies conducted in Europe from 2008 to 2017 addressing either knowledge or attitudes by the public towards the consent system, including the involvement of the family in the decision-making process, for post-mortem organ retrieval. Screening and data collection (...) were performed by two or more independent reviewers for each record. -/- Results: Of the 1482 results, 467 studies were assessed in full-text form, and 33 were included in this synthesis. When the deceased has not expressed any preference, a majority of the public support the family's role as a surrogate decision-maker. When the deceased expressly consented, the respondents' answers depend on whether they see themselves as potential donors or as a deceased's next-of-kin. Answers also depend on the relationship between the deceased and the decision-maker(s) within the family, and on their ethnic or cultural background. -/- Conclusions: Public views on the authority of the family in organ donation decision-making require further research. A common conceptual framework and validated well-designed questionnaires are needed for future studies. The findings should be considered in the development of Government policy and guidance regarding the role of families in deceased organ donation. (shrink)

Virtue-based approaches have attracted significant recent interest in argumentation, including a recent anthology of Chinese translations of important articles in the field. In this article, adapted from the introduction to that anthology, we discuss the origins of virtue argumentation and some of the challenges it has faced, as well as attempt to provide an overview of recent work on the virtues and vices relevant to argumentation. In the final section we discuss the articles that were selected and motivate their selection.

A naturalistic theory of intentionality is proposed that differs from previous evolutionary and tracking theories. Full-blown intentionality is constructed through a series of evolvable refinements. A first, minimal version of intentionality originates from a conjectured internal process that estimates an organism’s own fitness and that continually modifies the organism. This process produces the directedness of intentionality. The internal estimator can be parsed into intentional components that point to components of the process that produces fitness. It is argued that such intentional (...) components can point to mistaken or non-existing entities. Different Fregian senses of the same reference correspond to different components that have different roles in the estimator. Intentional components that point to intentional components in other organisms produce directedness towards semi-abstract entities. Finally, adding a general, population-wide means of communication enables intentional components that point to fully abstract entities. Intentionality thus naturalized has all of its expected properties: being directed; potentially making errors; possibly pointing to non-existent, abstract, or rigid entities; capable of pointing many-to-one and one-to-many; distinguishing sense and reference; having perspective and grain; and having determinate content. Several examples, such as ‘swampman’ and ‘brain-in-a-vat’, illustrate how the theory can be applied. (shrink)

The causal efficacy of a material system is usually thought to be produced by the law-like actions and interactions of its constituents. Here, a specific system is constructed and explained that produces a cause that cannot be understood in this way, but instead has novel and autonomous efficacy. The construction establishes a proof-of-feasibility of strong emergence. The system works by utilizing randomness in a targeted and cyclical way, and by relying on sustained evolution by natural selection. It is not vulnerable (...) to standard arguments against strong emergence, in particular ones that assume that the physical realm is causally closed. Moreover, it does not suffer from epiphenomenalism or causal overdetermination. The system uses only standard material components and processes, and is fully consistent with naturalism. It is discussed whether the emergent cause can still be viewed as ‘material’ in the way that term is commonly understood. (shrink)

According to moral testimony pessimists, the testimony of moral experts does not provide non-experts with normative reasons for belief. Moral testimony optimists hold that it does. We first aim to show that moral testimony optimism is, to the extent such things may be shown, the more natural view about moral testimony. Speaking roughly, the supposed discontinuity between the norms of moral beliefs and the norms of non-moral beliefs, on careful reflection, lacks the intuitive advantage that it is sometimes supposed to (...) have. Our second aim is to highlight the difference in the nature of the pragmatic reasons for belief that support moral testimony optimism and moral testimony pessimism, setting out more clearly the nature and magnitude of the challenge for the pessimist. A recently developed computational and neurobiological theory of phenomenal consciousness is applied to a series of persistent philosophical problems of consciousness (in recent formulations by Tye, Searle, and Chalmers). The book is an “introductory” reconstruction of Davidson on interpretation —a claim to be taken with a grain of salt. Writing introductory books has become an idol of the tribe. This is a concise book and reflects much study. It has many virtues along with some flaws. Ramberg assembles themes and puzzles from Davidson into a more or less coherent viewpoint. A special virtue is the innovative treatment of incommensurability and of the relation of Davidson’s work to hermeneutic themes. The (...) weakness comes in a certain unevenness. While generally convincing and well written, the book has low points which may leave the reader confused or unconvinced. Davidson is the hero in this book, and our hero is sometimes over idealized. A new theory that naturalizes biological function is explained and compared with earlier etiological and causal role theories. Etiological theories explain functions from how they are caused over their evolutionary history. Causal role theories analyze how functional mechanisms serve the current capacities of their containing system. The new proposal unifies the key notions of both kinds of theories, but goes beyond them by explaining how functions in an organism can exist as factors with autonomous causal efficacy. The goal-directedness and normativity (...) of functions exist in this strict sense as well. The theory depends on an internal physiological or neural process that mimics an organism’s fitness, and modulates the organism’s variability accordingly. The structure of the internal process can be subdivided into subprocesses that monitor specific functions in an organism. The theory matches well with each intuition on a previously published list of intuited ideas about biological functions, including intuitions that have posed difficulties for other theories. Bestaande literatuur over Knoet de Grote zijn troonsbestijging, zoals dat van vooraanstaand 19e-eeuws historicus Edward Augustus Freeman, zou een ‘geromantiseerd’ beeld hebben geschetst van de kwestie. Dit werkstuk zal kijken of dit beeld, aan de hand van Webers theorie over waar (...) legitimiteit in gegrond kan zijn, ten eerste terecht te noemen is en, ten tweede, of het Knoets status als legitiem vorst van het ‘Angelsaksische’ volk goed kan verklaren. A number of experiences have demonstrated how digital solutions are effective in improving quality of life (QoL) and health outcomes for older adults. Smart Health Age-Friendly Environments (SHAFE) is a new concept introduced in Europe since 2017 that combines the concept of Age-Friendly Environments with Information Technologies, supported by health and community care to improve the health and disease management of older adults and during the life-course. This chapter aims to provide an initial overview of the experiences available not only (...) in Europe, based on the research work of the participants of the International Interdisciplinary Network on Health and Well-being in an Age-Friendly Digital World (NET4Age-Friendly), which could be of interest to preventive, health and social authorities. The chapter reports good practices, pain points, and bottlenecks that may require a collaborative, interdisciplinary research approach to facilitate the transformations towards smart, sustainable, health and age-friendly cities and communities. Democratic societies expect their armed forces to act in a morally responsible way, which seems a fair expectation given the fact that they entrust their armed forces with the monopoly of violence. However, this is not as straightforward and unambiguous as it sounds. Present-day military practices show that political assignments, social and cultural contexts, innovative technologies and organisational structures, present military personnel with questions and dilemma’s that can have far-reaching consequences for all involved – not in the last place for (...) the soldiers themselves. A thorough training and education, in which critical thinking is developed and stimulated, seems therefore a necessary condition for morally responsible behaviour. This book aims to contribute to this form of ‘reflective practitioning’ in military practice. (shrink)

Recently developed extensions of evolutionary theory are used to explain the human self as an evolved, unitary, and purposeful phenomenon. A basic mechanism that can generate life's agency and goal-directedness is combined with mechanisms that can account for awareness by and of the self, and for the social characteristics of humans. The new theory is largely consistent with major existing theories of the self, in particular theories centred on self-esteem, self-determination theory, and terror management theory. It can therefore be regarded (...) as a meta-theory that brings these theories, and related ones, within a common evolutionary framework. The theory suggests two primary dimensions of the self, the depth of awareness of the self and the social extent of the self. The origin and development of consciousness is poorly understood. Although it is clearly a naturalistic phenomenon evolved through Darwinian evolution, explaining it in terms of physicochemical, neural, or symbolic mechanisms remains elusive. Here I propose that two steps had to be taken in its evolution. First, living systems evolved an intrinsic goal-directedness by internalizing Darwinian fitness as a self-estimated fitness. The self-estimated fitness participates in a feedback loop that effectively produces intrinsic meaning in the organism. Second, animals with advanced nervous (...) systems evolved a special form of communication that modifies the way each partner estimates fitness. The resulting change in intrinsic meaning is experienced subjectively as a primary form of consciousness. This primary form is subsequently used to generate, partly through internalized dialogue, more complex forms of consciousness, such as consciousness of the natural and social worlds, consciousness of the self, and language-dependent forms of consciousness. PUERPÉRIO EM BOVINOS -/- INTRODUÇÃO -/- O puerpério é definido como o período entre o parto e a apresentação do primeiro estro fértil. Dois processos ocorrem durante o puerpério: a involução uterina e o início da atividade ovariana pós-parto. Em vacas leiteiras, os cuidados médicos pós-parto são essenciais nos programas de manejo, uma vez que as patologias uterinas são diagnosticadas e tratadas nesse período para que a vaca esteja em ótimas condições para ser inseminada, uma vez terminado o período de (...) espera voluntário. -/- 4.1 Involução uterina -/- O útero após o parto sofre modificações macroscópicas e microscópicas, até atingir as características de um útero não gestante, o que leva de 30 a 45 dias. O peso e o tamanho pós-parto diminuem rapidamente como consequência da atrofia das fibras musculares; por necrose das carúnculas e por eliminação de líquidos. Ao mesmo tempo em que o útero diminui de tamanho, o endométrio passa por um processo regenerativo para poder hospedar uma nova gestação. -/- A involução é promovida pelas contrações uterinas, que facilitam a eliminação de fluidos e dejetos e reduzem o tamanho do útero. As contrações são causadas pela secreção contínua de PGF2α, de origem uterina, e pela ocitocina secretada durante a amamentação. A PGF2α é secretada durante as primeiras 3 semanas pós-parto e seu envolvimento é considerado necessário para que a involução uterina ocorra normalmente. -/- Durante a involução uterina, as secreções conhecidas como lóquios, que são formadas por restos de membranas, carúnculas, fluidos fetais e sangue, são derramadas pela vagina. Essas secreções variam da cor vermelha ao marrom, têm consistência viscosa e são inodoras. A maioria dos lóquios é descartada durante os primeiros 15 dias após o parto e, em seguida, praticamente desaparece, exceto em casos de involução uterina anormal, quando o útero continua a derramar fluidos de consistência, cor e odor diferentes (figura 1). -/- Figura 1: O útero após o parto sofre alterações macroscópicas e microscópicas, até atingir as características de um útero não grávido, o que leva de 30 a 45 dias. -/- Antes do parto, o útero é estéril, pois é protegido da contaminação bacteriana pelo colo do útero ou cérvix. Durante e após o parto, essa barreira física desaparece e o útero é invadido por bactérias que se encontram no meio ambiente, pele e fezes, ou que são introduzidas durante a assistência ao parto. Além disso, a capacidade funcional dos fagócitos uterinos é baixa após o parto, o que contribui para o estabelecimento de infecções. Cerca de 95% das vacas desenvolvem infecções uterinas durante a involução, e uma alta porcentagem de vacas as eliminam por meio de mecanismos naturais. -/- Os mecanismos de defesa uterinos são constituídos pelas barreiras anatômicas (vagina, vulva e cérvix); fatores fisiológicos (produção de muco na vagina e cérvix); fagocitose promovida por neutrófilos, que migram da circulação geral para o útero; pela produção de substâncias inespecíficas, que inibem o crescimento bacteriano e favorecem a eliminação de microrganismos. A capacidade de eliminar bactérias do útero é determinada pelos hormônios ovarianos (progesterona e estrogênios); durante o diestro, a progesterona reduz a migração de neutrófilos, suprime o sistema imunocompetente, fecha o colo do útero e causa atonia uterina, resultando em aumento da suscetibilidade à infecção. Ao contrário, durante a fase folicular do ciclo estral (proestro e estro), o estradiol promove a migração de neutrófilos, abre a cérvix e aumenta o tônus uterino, o que facilita a eliminação de agentes infecciosos. Além disso, a atividade dos neutrófilos é afetada pelas deficiências antioxidantes e pela profundidade do balanço energético negativo. -/- 4.2 Anormalidades do puerpério -/- Após o parto, ocorrem algumas patologias que retardam a involução uterina e, consequentemente, afetam o intervalo entre o parto e o primeiro serviço. -/- 4.2.1 Retenção da placenta -/- A placenta é eliminada dentro de 12 horas após o parto; a retenção da placenta por mais de 24 horas é considerada uma patologia. A retenção placentária (RP) é uma alteração frequente do puerpério, que deve ser considerada como um sinal clínico de várias condições que podem ter origem infecciosa (aborto), metabólica (hipocalcemia, cetose, síndrome do vaca gorda), deficiências nutricionais (selênio e vitamina E) e erros de manejo (demasiada intervenção no parto). -/- A incidência de RP varia de 5 a 15% e depende, em grande parte, do estado de saúde e do manejo do rebanho. A RP é o principal fator de risco para infecções uterinas (metrite puerperal, metrite, endometrite e endometrite subclínica); também causa atraso no período entre o parto e a concepção e está associada à redução do percentual de concepção no primeiro serviço. -/- Em termos econômicos, determinou-se que a RP causa perdas significativas principalmente devido aos custos de serviços médicos, aumento da taxa de eliminação e diminuição da fecundidade. Além disso, vacas com retenção placentária produzem 355 kg a menos de leite durante os primeiros 60 dias do que vacas que não apresentam esta patologia (figuras 2 e 3). -/- 4.2.2 Patogenia -/- A placenta está ligada ao endométrio pelas vilosidades coriônicas, que estão ligadas às carúnculas (junção carúncula-cotilédone). A união entre essas estruturas é favorecida por um fluido adesivo formado por colágeno e outras proteínas. Antes e durante o trabalho de parto, observa-se um aumento da atividade das enzimas proteolíticas (colagenase, talvez a mais importante), que são responsáveis por separar o cotilédone da carúncula. Posteriormente, a placenta é removida mecanicamente por contrações uterinas após o parto do feto. -/- A etiologia e patogênese da retenção placentária não são conhecidas. Uma explicação proposta é que isso se deve a uma falha dos mecanismos proteolíticos responsáveis pela separação do cotilédone da carúncula. Foi observado que a atividade da colagenase no cotilédone é maior em vacas que não retêm placenta do que naquelas que a retêm. Propõe-se que alguns fatores de risco para retenção placentária possam estar atuando na redução da atividade da colagenase. Por outro lado, as condições inflamatórias na junção carúncula-cotilédone de origem infecciosa também impedem a separação da placenta. -/- Figura 3: Vaca com retenção placentária. -/- Figura 2: Vaca com retenção placentária. Em rebanhos comerciais, 5 a 15% das vacas retêm a placenta. -/- 4.2.3 Tratamento -/- Existem vários tratamentos, como a remoção manual da placenta em combinação com a aplicação local de antibióticos (infusões intrauterinas) e a administração de produtos hormonais (ocitócicos e PGF2α). A eficácia desses tratamentos é discutível. -/- A remoção manual da placenta é o tratamento mais popular; entretanto, não é a melhor, pois causa danos ao endométrio, que variam de leve sangramento a hematomas, mesmo quando não há evidências externas. Além disso, a remoção manual diminui a capacidade fagocítica dos leucócitos uterinos, resultando em metrite mais grave, maior atraso na involução uterina e baixo desempenho reprodutivo. -/- Outro tratamento consiste em cortar a placenta ao nível da vulva. Posteriormente, quando a placenta se separa das carúnculas, devido ao processo de decomposição do tecido, uma leve tração da vulva é suficiente para retirá-la, sem consequências. Essas vacas devem ser observadas quanto à febre e imediatamente integradas ao programa de check-up pós-parto, pois certamente desenvolverão metrite ou endometrite. A administração de antibióticos, tanto nos casos de retirada manual como naqueles em que a placenta é cortada, depende do estado geral da vaca. Deve-se sempre ter em mente que a RP é o principal fator de risco para metrite, portanto as vacas devem ser observadas para detectar prontamente aquelas que apresentam febre. Os antibióticos devem ser considerados para inibir a putrefação das membranas fetais, o que pode atrasar sua expulsão. No caso de opção pela administração de antibióticos, deve-se optar pela via parenteral em vez da intrauterina, pois esta causa irritação do endométrio, que está associada à baixa fertilidade (figura 4). -/- Outros tratamentos são baseados na administração de hormônios que estimulam a mobilidade uterina (ocitocina, estrogênios e PGF2α); entretanto, não há evidências clínicas que demonstrem sua eficácia; além disso, a causa menos frequente de RP é a incapacidade mecânica do útero de expelir a placenta. -/- 4.2.4 Prevenção -/- As estratégias de prevenção dessa patologia devem ter como objetivo reduzir a influência dos fatores de risco para RP. Assim, a incidência de abortos deve ser reduzida por meio de programas eficazes de vacinação e biossegurança; deve-se evitar períodos de seca muito longos para que as vacas não cheguem obesas ao parto; intervir o mínimo possível nos partos, e se atendido, deve ser feito com medidas de higiene rígidas; oferecer sais minerais de boa qualidade e administrar antioxidantes antes do parto (vitamina E, selênio e betacarotenos; tabela 1). -/- Figura 4: A placenta não deve ser removida inserindo a mão na vagina. Adaptado de RUIZ et al., 2008. -/- 4.3 Infecções uterinas -/- Cerca de 95% das vacas desenvolvem infecção uterina durante os primeiros dias pós-parto; no entanto, a maioria elimina infecções por meio de seus mecanismos de defesa e apenas 30 a 50% delas desenvolvem metrite ou endometrite nas primeiras três semanas. As bactérias mais frequentes encontradas em processos inflamatórios no útero são: Arcanobacterium pyogenes (antes Actinomyces pyogenes), Fusobacterium necrophorum e Escherichia coli. Essas três bactérias agem sinergicamente. -/- 4.3.1 Metrite puerperal -/- A metrite puerperal é comum em vacas com retenção placentária. Essa patologia é observada nas três primeiras semanas pós-parto e é caracterizada pela presença de secreções abundantes na luz uterina que são vermelhas ou marrons, aquosas, fétidas e com involução retardada. As vacas afetadas mostram sinais de doença sistêmica (toxemia e febre > 39,5 °C) e diminuição da produção de leite (figura 5). -/- Figura 5: Vaca com metrite puerperal; vacas afetadas apresentam sinais de doença sistêmica. -/- 4.3.2 Metrite clínica -/- Metrite é o processo inflamatório que envolve as diferentes camadas do útero (mucosa, muscular e serosa). Esta condição se apresenta nos primeiros 21 dias pós-parto e é caracterizada por atraso na involução uterina e secreção purulenta, e não há sinais de doença sistêmica (figura 6). -/- 4.3.3 Endometrite clínica -/- Endometrite se refere à inflamação do revestimento uterino; clinicamente, caracteriza-se por retardo na involução uterina e eliminação de exsudato purulento ou mucopurulento. Pode surgir nos primeiros 21 dias pós-parto ou mais, sem apresentar qualquer afetação no estado geral. -/- Figura 6: Vaca com exsudato mucopurulento. Na prática, é difícil estabelecer um diagnóstico diferencial entre endometrite e metrite. Uma vaca com essa secreção pode ser tratada com cefalosporinas intrauterinas ou, se ela já estiver em ciclo, a administração de PGF2a é eficaz. -/- 4.3.4 Endometrite subclínica -/- Apresenta-se entre 21 e 40 dias pós-parto, não apresenta sinais externos e só é diagnosticada pela citologia uterina. Essa condição afeta entre 20 e 30% das vacas. Os fatores de risco identificados são retenção placentária e metrite. -/- 4.4 Diagnóstico -/- O diagnóstico é baseado na avaliação uterina por palpação retal, na qual são verificados o grau de involução e as características das secreções. Além disso, é necessária uma avaliação clínica geral, uma vez que vacas com metrite nos primeiros 10 dias pós-parto desenvolvem febre. Outra forma de estabelecer o diagnóstico é avaliando as secreções uterinas sem palpação retal. Pode ser feito introduzindo a mão por via vaginal, após a limpeza da região; embora este método possa parecer arriscado, a experiência em campo indica que é um método seguro e rápido. Outra possibilidade é por meio da vaginoscopia, esse método permite a avaliação da cérvix e das secreções uterinas. Existe um instrumento (Metricheck) que é inserido pela vagina e possui uma campânula atraumática na extremidade que permite a coleta das secreções uterinas (figuras 7 e 8). -/- 4.5 Impacto das infecções uterinas na reprodução e produção -/- As infecções uterinas (metrite e endometrite) prolongam o período do parto até a concepção; diminuem a porcentagem de concepção no primeiro serviço; a porcentagem de resíduos aumenta; a taxa de vacas inseminadas diminui e a produção diminui (300 kg a menos que vacas não afetadas). Além disso, eles afetam o período do parto à primeira ovulação; em vacas com puerpério anormal, os folículos dominantes são menores em diâmetro e produzem menos estradiol do que vacas saudáveis. Também uma proporção menor dos folículos da primeira onda ovula em comparação com as vacas com puerpério anormal. -/- A endometrite subclínica diminui a taxa de concepção no primeiro serviço e aumenta os dias abertos. Além disso, a metrite ou endometrite pode afetar os ovidutos e causar inchaço, obstrução e aderências (esse processo é mais frequente em vacas que recebem injeções de estrogênio). -/- Figura 8: Outra forma de diagnosticar infecções uterinas é por meio da introdução de um instrumento que possui em sua extremidade uma campânula atraumática que permite a coleta de secreções uterinas (Metricheck). -/- Figura 7: O diagnóstico de infecções uterinas pode ser estabelecido com a introdução da mão por via vaginal, após limpeza da região. -/- 4.6 Tratamentos -/- Para evitar o efeito negativo das infecções uterinas na eficiência reprodutiva, é necessário um diagnóstico e tratamento oportunos. Existem diversos tratamentos para a metrite ou endometrite, como o uso de antibióticos sistêmicos ou intrauterinos, infusões intrauterinas de antissépticos e a administração de hormônios. -/- 4.6.1 Antibióticos -/- Os tratamentos com antibióticos intrauterinos são usados há muitos anos e são uma opção, desde que alguns aspectos sejam considerados: que o útero é um ambiente anaeróbio; que há a presença de exsudatos e tecidos em decomposição e a existência de uma grande diversidade de bactérias, algumas das quais até produzem enzimas que inativam alguns antibióticos. -/- Como o útero é um ambiente anaeróbio, os antibióticos do grupo dos aminoglicosídeos não são ativos, pois precisam de oxigênio. Por outro lado, o acúmulo de exsudato purulento e restos de tecido no útero inibe a atividade das sulfonamidas. Os nitrofuranos são eficazes contra A pyogenes, somente quando doses extremamente altas são usadas; as doses usuais nunca atingem a concentração inibitória mínima no endométrio; além disso, não são ativos na presença de sangue ou exsudato purulento, são irritantes e estão associados a problemas de fertilidade. -/- A penicilina intrauterina ou parenteral é eficaz na cura de infecções entre os dias 25 e 30 após o parto; ou seja, quando for observada uma diminuição na diversidade de espécies bacterianas (menor probabilidade de que algumas bactérias produzam penicilinase) e predomina A pyogenes, que é sensível a este antibiótico. As formulações intrauterinas de cefalosporinas (cefapirina benzatina) são eficazes em vacas com endometrite entre os dias 15-20 pós-parto e não é necessário retirar o leite do mercado. -/- A tetraciclina é o antibiótico mais utilizado devido ao seu amplo espectro e por manter sua atividade nas condições do útero pós-parto; porém, a probabilidade de resistência bacteriana é alta devido ao seu uso contínuo por muitos anos, também causa danos ao endométrio e reduz a fertilidade (figura 9). -/- Em testes de campo, a administração de uma única infusão intrauterina de cefapirina benzatina, em vacas com endometrite entre os dias 15 a 20 pós-parto, produz uma maior taxa de recuperação e não afeta a fertilidade, em comparação com aquelas que foram tratadas com infusões de oxitetraciclina. -/- O problema mais importante na terapia com antibióticos é estabelecer um critério de quais animais realmente precisam. Nos casos de metrite puerperal não há dúvida, essas vacas precisam de tratamento com antibióticos por via sistêmica e intrauterina. Porém, em casos de metrite e endometrite a decisão é difícil, pois muitas vacas se curam sem nenhum tratamento. Na prática, antes que os antibióticos sejam administrados às vacas, deve-se considerar alguns dos fatores: características das secreções uterinas, dias após o parto, início da atividade ovariana, presença de febre e condição corporal. -/- Figura 9: As infusões de oxitetraciclina causam irritação do endométrio e diminuem a fertilidade. -/- 4.6.2 Tratamento com infusões de substâncias antissépticas -/- Tratamentos baseados na administração intrauterina de substâncias antissépticas também são usados. O tratamento mais frequente consiste na infusão de soluções de iodo; no entanto, os resultados com esta terapia não são positivos e estão associados a uma diminuição subsequente da fertilidade. Deve-se ter cautela no uso de antissépticos intrauterinos, pois todos causam irritação, afetam os mecanismos de defesa do útero e, em alguns casos, chegam a causar necrose do endométrio. -/- 4.6.3 Tratamentos hormonais -/- Na prática, a administração de estrogênio é comum em casos de metrite, principalmente quando o útero retém muito líquido. Sabe-se que os estrógenos em condições fisiológicas favorecem a contratilidade uterina e auxiliam na eliminação de infecções, porém, em doses farmacológicas o efeito é negativo. A administração de estrogênios pode contribuir para infecções que ascendem aos ovidutos e causam salpingite, aderências ovarianas e infertilidade. -/- A PGF2α desempenha um papel importante no parto e durante a involução uterina. Em vacas com puerpério normal, a duração dos níveis elevados de PGF2α está negativamente correlacionada com o tempo de involução uterina; portanto, quanto mais tempo duram os níveis elevados de PGF2α, menor é o tempo de involução uterina. Além disso, a administração de PGF2α a cada 12 horas, dos dias 3 a 10 pós-parto, encurta o período de involução uterina. Ao contrário, em vacas com puerpério anormal, foi observada uma correlação positiva entre as concentrações de PGF2α e o tempo de involução uterina; portanto, vacas com retenção placentária ou endometrite apresentam níveis mais elevados de PGF2α. -/- Algumas evidências envolvem a PGF2α como parte do mecanismo de eliminação da placenta; ou seja, foi observado que as vacas que normalmente eliminam a placenta apresentaram maiores concentrações de PGF2α na placenta, em comparação com as vacas com retenção placentária. Foi proposto que a falha do processo que leva à separação do cotilédone da carúncula está associada a uma alteração no metabolismo da prostaglandina, resultando em uma diminuição na PGF2α e um aumento na PGE2. Por outro lado, a inibição da síntese de PGF2α após o parto, por meio da injeção de ácido acetilsalicílico (aspirina), causa retenção da placenta. Esse conhecimento motivou estudos nos quais o tratamento com PGF2α após o parto foi avaliado para prevenir a retenção placentária ou reduzir o tempo de involução em vacas com retenção placentária ou metrite. Em alguns estudos, uma única injeção de PGF2α foi administrada nas primeiras horas após o parto com resultados conflitantes. Em outros estudos, a injeção de PGF2α entre os dias 20 a 30 pós-parto foi avaliada e não foi observado um efeito favorável na involução uterina. Porém, a injeção sistemática de PGF2α, a cada 14 dias a partir do 25º dia pós-parto, aumenta a proporção de vacas inseminadas, o que aumenta a taxa de prenhez. A PGF2α causa contrações uterinas e favorece a eliminação de secreções; entretanto, um efeito favorável da PGF2α na involução uterina, sem envolver um corpo lúteo, é questionável. -/- Na prática, um programa é usado com base na administração sistemática de PGF2α a todas as vacas a cada 14 dias, do 25º ao 30º dia pós-parto. Esse tratamento é baseado no encurtamento das fases lúteas, o que favorece a eliminação das infecções. Deve-se observar que cerca de 30% das vacas desenvolvem corpos lúteos longevos (21 a 50 dias) nos primeiros ciclos pós-parto, nessas condições a injeção de PGF2α, a cada 14 dias, encurta o ciclo estral e diminui o risco de persistência de infecções uterinas. -/- 4.7 Piometra -/- Esta patologia se desenvolve em vacas que ovulam nos primeiros 20 dias pós-parto e, concomitantemente, sofrem de infecção uterina. Nessas condições, a progesterona promove a proliferação bacteriana e fecha o colo do útero, causando acúmulo de exsudato purulento no útero. As alterações causadas no endométrio alteram a secreção de PGF2α, resultando na persistência do corpo lúteo e anestro. Vacas com piometra respondem muito bem ao tratamento com PGF2α; uma segunda injeção desse hormônio, 14 dias depois, encurta o período de recuperação (figura 10). -/- Figura 10: A piometra é caracterizada pelo acúmulo de exsudato purulento no útero, colo uterino fechado e presença de corpo lúteo. O diagnóstico é feito por palpação retal e tratado com PGF2α. -/- 4.8 Vaginite em fêmeas gestantes -/- A vaginite em fêmeas grávidas ocorre com baixa frequência; porém, quando há um caso, causa alarme porque é confundido com um aborto. Nesses casos, o veterinário deve fazer um exame retal para determinar se o produto está vivo e se a cérvix está aberta. O diagnóstico é concluído com o exame vaginal com vaginoscópio ou diretamente à mão com luva limpa. -/- No caso de um aborto, o produto geralmente está morto e a cérvix está aberta; nestes casos, o produto deve ser extraído. Ao contrário, se a cérvix estiver fechada e o feto estiver vivo, o prognóstico é favorável e o problema diminui com o tratamento com antibióticos sistêmicos. -/- RESUMO -/- < >O útero após o parto pesa 13 kg e no dia 21 pós-parto, 1 kg. A regeneração do epitélio uterino leva 25 dias. A regeneração das camadas profundas do endométrio leva 42 a 56 dias. 95% das vacas desenvolvem alguma infecção uterina. 30 a 50% das vacas desenvolvem metrite nas primeiras duas semanas após o parto. 15 a 20% persistem como endometrite clínica > 3 semanas após o parto. 18 a 20% têm metrite com efeitos sistêmicos. 20 a 30% das vacas têm endometrite subclínica. 5 a 15% das vacas retêm a placenta. 355 kg de leite são perdidos em vacas com retenção placentária. Em vacas que sofrem de puerpério anormal, a proporção de vacas prenhes é reduzida em 10 por cento no 90º dia pós-parto. As the Zika virus pandemic continues to bring worry and fear to health officials and medical scientists, Centers for Disease Control and Prevention (CDC) and World Health Organization (WHO) have recommended that residents of the Zika-infected countries, e.g., Brazil, and those who have traveled to the area should delay having babies which may involve artificial contraceptive, particularly condom. This preventive policy, however, is seemingly at odds with the Roman Catholic Church’s position on the contraceptive. As least since the promulgation of (...) Paul VI’s 1968 encyclical, Humanae Vitae, the Church has explicitly condemned artificial birth control as intrinsic evil. However, the current pontiff, Pope Francis, during his recent visit to Latin America, remarked that the use of artificial contraception may not be in contradiction to the teaching of Humanae Vitae while drawing a parallel between the current Zika Crisis and the 1960’s Belgian Congo Nun Controversy. The pope mentioned that the traditional ethical principle of the lesser of two evils may be the doctrine that justified the exceptions. The authors of this paper attempt to expand the theological rationale of the pope’s suggestion. In so doing, the authors rely on casuistical reasoning as an analytic tool that compares the Belgian Congo Nun case and the given Zika case, and suggest that the former is highly similar to, if not the same as, the latter in terms of normative moral feature. That is, in both cases the use of artificial contraception is theologically justified in reference to the criteria that the doctrine of the lesser of two evils requires. The authors wish that the paper would provide a solid theological-ethical ground based on which condom-use as the most immediate and effective preventive measure can be recommended in numerous Catholic hospitals as well as among Catholic communities in the world, particularly the most Zika-affected and largest Catholic community in the world, Brazil – 123 million present Brazilian citizens are reported to be Roman Catholic. The Centre for Society and Genomics (CSG) was established in 2004, funded by NGI (the Netherlands Genomics Initiative). Funding was continued in 2008. This report summarises the basic outcomes of almost a decade of interactive societal research, in close collaboration with the other centres of the NGI network. There are two reasons for presenting these results. First of all, at the end of this year, the CSG Next programme (2008-2013), encompassing more than 50 research projects conducted at 10 Dutch universities, (...) will be completed. Moreover, we are currently preparing ourselves for the years to come. The network of principal investigators, together with the research communities they represent and the societal and international networks they are involved in, have agreed to continue to work together, on the basis of mutual learning, transdisciplinary collaboration and collegial support. Notably, we offer our networks, experiences and expertise to help prepare the ground for promoting Responsible Research and Innovation (RRI) in the context of Horizon2020, together with our European colleagues. This report summarises what our type of research can achieve and how we want to continue our activities in the future. After a concise sketch of the life sciences landscape as it has evolved during the past seven decades or so, we explain how CSG came about and what kind of approach we have developed. Subsequently, we list our main results, notably in the form of project vignettes, so as to make the harvest of the CSG Next programme as tangible and concrete as possible. Finally, we explain how we see our role in the future. As is already indicated by the title: this is not merely a retrospective summary of our results (CSG harvest), but an invitation to readers (from academia, industry, policy and civil society) to reassemble and to optimally prepare ourselves for things to come, by strengthening and broadening our collaborative efforts, building on what we have achieved so far. Background: Critically ill patients often require complex clinical care by highly trained staff within a specialized intensive care unit (ICU) with advanced equipment. There are currently limited data on the costs of critical care in low-and middle-income countries (LMICs). This study aims to investigate the direct-medical costs of key infectious disease (tetanus, sepsis, and dengue) patients admitted to ICU in a hospital in Ho Chi Minh City (HCMC), Vietnam, and explores how the costs and cost drivers can vary between the (...) different diseases. -/- Methods: We calculated the direct medical costs for patients requiring critical care for tetanus, dengue and sepsis. Costing data (stratified into different cost categories) were extracted from the bills of patients hospitalized to the adult ICU with a dengue, sepsis and tetanus diagnosis that were enrolled in three studies conducted at the Hospital for Tropical Diseases in HCMC from January 2017 to December 2019. The costs were considered from the health sector perspective. The total sample size in this study was 342 patients. -/- Results: ICU care was associated with significant direct medical costs. For patients that did not require mechanical ventilation, the median total ICU cost per patient varied between US$64.40 and US$675 for the different diseases. The costs were higher for patients that required mechanical ventilation, with the median total ICU cost per patient for the different diseases varying between US$2,590 and US$4,250. The main cost drivers varied according to disease and associated severity. -/- Conclusion: This study demonstrates the notable cost of ICU care in Vietnam and in similar LMIC settings. Future studies are needed to further evaluate the costs and economic burden incurred by ICU patients. The data also highlight the importance of evaluating novel critical care interventions that could reduce the costs of ICU care. (shrink)

In The Entangled Brain, Pessoa criticizes standard approaches in cognitive neuroscience in which the brain is seen as a functionally decomposable, modular system with causal operations built up hierarchically. Instead, he advocates for an emergentist perspective whereby dynamic brain networks are associated, not with traditional psychological categories, but with behavioral functions characterized in evolutionary terms. Here, we raise a number of concerns with such a radical approach. We ultimately believe that while much revision to cognitive neuroscience is welcome and needed, (...)
